Medicaid Primary Care Providers

The primary care providers (PCPs) shown in the map have accepted Medicaid and enrolled in Community Care of North Carolina (CCNC) as a PCP in the past. However, you may choose any doctor who accepts Medicaid to be your PCP.

  1. Call the doctor you want to be your PCP and ask if he or she is taking new Medicaid patients. 
  2. Call your local Department of Social Services case worker, who will update your records and order a new Medicaid ID card with your PCP’s name and medical home.
  3. You can use your current Medicaid ID card to visit your new PCP until the new one arrives in the mail.
